send it in, or drop us a line that includes the following
information:
- Your
name, address, daytime telephone number (optional), and
e-mail address (optional)
- Your
date of birth and the date of birth for your secondary beneficiary,
if any. Your gift benefits are partially determined by your
age
- Whether
you would like an example based on the minimum gift amount
or another amount
- Whether
you plan to fund your gift with cash or with appreciated
securities (include approximate cost of securities when
purchased and the date you purchased them)

information you provide will remain strictly confidential.
