FILING INSURANCE CLAIMS AT BARNARD COLLEGE
 
 1.    Be sure you have a Super Bill from the clinician or institution (e.g., emergency room, x-ray facility, laboratory) where you received care. A Super Bill gives the date you were seen, a specific diagnosis with a code, and the name of the clinician who treated you.
 
2.    Find out if you are covered by your family medical insurance. If yes., go to Step 3; if no, go to Step 6. For reimbursements for a visit to Columbia Health Services, go to Step 6. You do NOT need to file with your family insurance.

3.    Submit one copy of the Super Bill with the claim form from your family insurance company  to the indicated address, filing out the requested information. If they ask for date of service, diagnosis, etc., you can put "Please see attached bill." BE SURE TO SIGN SO THAT YOUR CLINICIAN CAN BE PAID DIRECTLY.

4.    You will receive notification from your family insurance company   telling you how much of the bill has been paid. If your bill has not been totally paid, proceed to Step 5.
 
5.    Submit one copy of the Super Bill and a copy of the explanation of payment from your family insurance company or Medicaid, and a Barnard insurance form (available in Health Services) to the address listed on the Barnard insurance form.
 
6.    If the Barnard insurance is your only coverage, submit one copy of the Super Bill or your receipt from CUHS and the Barnard insurance form (available in Health Services) to the address listed on the Barnard form. Fill in the requested information. For diagnosis, date of service, etc., you can put "Please see attached bill." If you have only a receipt, you must fill out this information. Except for visits to Columbia Health Services, you must complete the section about other/family insurance or your claim will not be processed.
 
BE SURE TO KEEP A COPY OF EVERYTHING YOU SUBMIT IN CASE THERE ARE PROBLEMS, OR YOUR CLAIM IS LOST.
 
Please contact the Health Service if there are problems getting your claim paid.
 
REMEMBER, payment for emergency room visits is guaranteed when referred by the clinician on call. (Otherwise visits deemed non-emergent may be denied by the insurance carrier.) The clinician on call is available 24 hours/day whenever Health Services is closed at 866-966-7788 throughout the academic year, including Thanksgiving, Winter and Spring Breaks.
 
Referral to a Specialist:
What You Need to Know about Insurance

You are being referred to a specialist. A combination of family and Barnard insurance will pay for your visit if you follow the steps below and on the back of this sheet.

Bring the following items with you to your appointment:

1)    A completed three-piece (triplicate) "Referral to Consultant" form from your Barnard clinician. If not attached, ask your clinician.

2)    A claim form for the Gerber Life Insurance Company (the Barnard College Student Injury and Sickness Insurance Plan.) If not attached, pick up at the reception desk or ask your clinician for one.

3)    If you are covered by family insurance, an insurance member ID card and a claim form from your family or family insurance policy, signed by the policyholder (i.e., your parent) if required by that insurance company.

4)    Approval from the Director of Health Services if you are being referred for surgery.

Remember…

If your family insurance required that see only physicians in their network, you should schedule your appointment with them or obtain the appropriate authorizations from them well in advance to see a physician who is "out of network."

If you are not sure if your family insurance will cover you visit, you will need to find out prior to making your appointment. You still must submit your bills to your family insurance before sending them to Barnard/Student Resources/Gerber Life Insurance Company. You will receive an Explanation of Benefits (EOB) from them stating how much, if anything, they paid.

Balances left over after family insurance has paid portion will be evaluated by the student insurance if the referral was approved by a Barnard clinician and a claim is properly filed by you with the Barnard insurance, usually within 90 days of treatment.

If you are not covered by family insurance, or that coverage has high deductibles, or is geographically limited and will not cover you while you are in New York, we strongly encourage you to elect the optional supplemental benefit plan.  (If you will not be registered for classes in the Spring, you will not be eligible for spring coverage or the summer extension.)

Supplemental Insurance Plans
 
Fall 2005 Optional Benefit Plan:
Coverage starts from August 22, 2005 to January 16, 2006 at
a cost of $44.
 
Spring 2006 Optional Benefit Plan:
Coverage starts from January 17 to May 19 at a cost of $44
Deadline for enrollment is February 15
 
Summer 2006 Basic Benefits Extension Plan:
Coverage starts from May 20 to August 21 at a cost of $64
Deadline for enrollment is May 26
 
Summer 2006 Basic Benefits and Optional Benefit Plan Extension:
Coverage starts from May 20 to August 21 at a cost of $90
Deadline for enrollment is May 26

 

Supplemental insurance extends the basic coverage from
$5,000 to $50,000 for hospitalizations. Outpatient coverage for
bothBasic and Supplemental insurance is limited, but either plan will
reimburse you for sick visits to Columbia Health Services and give
you accident/sickness coverage anywhere in the U. S. an abroad.
Short term travel insurance is recommended.
