ODS Equipment Loan Agreement

The Barnard College Office of Disability Services (ODS) agrees to loan the following equipment/materials:

This agreement automatically ends at the completion of the current term for which a student is registered for at least one course at  Barnard, or  when a student withdraws from the course/s. Equipment/materials must be returned to ODS in proper working condition no later than the completion of final examinations or withdrawal from the course/s, whichever comes first. Extensions may be authorized in person, or in writing with prior approval from ODS only if all conditions set forth herein are included in the written request for extension. Persons borrowing equipment/materials understand that such materials and/or equipment are the property of  Barnard College.

By signing this agreement, the borrower accepts responsibility for loss, damage and/or failure to return borrowed equipment/materials in immediately reusable condition. Borrower will reimburse ODS for repairs or current replacement value of equipment/materials as determined by the vendor from which the equipment/materials were purchased or replaced.
