How to register with ODS
If you have a temporary disability and need support services, please call ODS @ 212/854-4634 to make an appointment as soon as possible. All students with either temporary or permanent disabilities are required to have appropriate disability documentation on file in ODS, known as the Disability Certification Form (DCF). The DCF may be completed by one of the SHS clinicians or by a student’s private physician. All students with temporary disabilities are encouraged to:
1. Check-in with the Student Health Services (SHS), if you have not already done so.
2. Notify your advisor and class dean of your situation and registration with ODS.
During the registration appointment, disability-related classroom and/or test accommodations will be discussed—and students should then self-identify their disabilities and accommodations to their instructors on an as-needed basis.
