Well, folks, it is that time of year again: time to reflect on your accomplishments as you finish up the semester.
Information on the Personnel Form and in the c.v. provides the basis for decisions about merit pay increases for tenured faculty. For untenured and off-ladder faculty, the form and c.v. track progress which is considered as re-appointment decisions are made. Thus, it is imperative that each member of the faculty submit copies of the form and an updated c.v. in a timely fashion.
Two copies of the completed form accompanied by c.v's. should be prepared.
Please send one to me, and one to your Chair.
The deadline for submission to this office and your chair is d/m/y.
[Assistant Professors may, if they wish, defer submission of their forms until d/m/y.]
